- As a courtesy to our patients, we make every effort to call
and remind you about the upcoming well check.
- Patients with well child exams who do not show up for their
appointment and do not call to cancel the scheduled visit time
will be charged $30. This is not covered by insurance.
- To avoid these charges, PLEASE call us to cancel your scheduled
appointment as soon as you can, but no later than 24 hours before
the appointment. This allows us time to fill the appointment time
with other children who need to be seen.
- With the high costs of postage, copying, paper and staff time,
we have found it necessary to begin charging for record copying
for transferred out patients.
- Treehouse Pediatrics will charge $15 per patient or $25 per
family for copies of medical records. This does not apply if a
copy of the immunization record only is requested.
- Copies of immunization records will continue to be provided
free of charge.
- If you are uninsured, have not met your deductible or if we
are unable to verify insurance coverage, we require the visit
to be paid in full at your first visit.
- At a later date, if we are able to submit the claim for payment
you will receive a refund for any overpayment you might have incurred.
- We will make every effort to work with your insurance and file
your insurance claims. In the event a charge has been applied
to the patient, we will send out a statement.
- All balances are required to be paid in full unless prior arrangements
have been made.
- Accounts balances that have been neglected will be assigned
to an outside collection agency. Additional penalties and interest
will be applied.
